House's newest patient is 18-year-old Jack (guest star Patrick Fugit, “Almost Famous”), brought to the hospital after experiencing a heart attack and massive vomiting. Barely an adult himself, Jack has been the parent to his younger brother and sister since their parents died. After a brief perusal of Jack's file, House thinks he's got the diagnosis, seals it in an envelope and turns the process into a game, challenging Cameron, Foreman and Chase to figure it out on their own. When Jack is given an option that would lead to a possible reprieve from his worsening illness, he's faced with a decision that could break up his little family forever. Meanwhile, in an attempt to extract a confession, Tritter (guest star David Morse) makes it almost impossible for Wilson to practice medicine, driving a friendship-ruining wedge between House and Wilson in the“Whac-A-Mole” episode of HOUSE.
今集House的病人是一名18歲的青年Jack。Jack任職於兒童餐廳，一日他跟舉辦生日會的兒童玩得興高采烈之際，因突發性心臟病和大量嘔 吐而昏倒被送往醫院。自從Jack的父母過世以後，他一直充當弟妹的家長。House把Jack的診斷結果放在信封內，要求他的隊員在短時間內憑自己的能 力找出Jack病因。
Jack雖然清醒，但要留院觀察，House前往探望。另一方面，隊員則努力地從House留下的提示努力找出Jack的病因。最後，Jack面對一個可以令病情好轉的機會，然而他所下的決定或會令他的家庭永遠破裂，到底他會如何取捨？
